Job Description
A bit about this role:
Devoted Medical is building clinical programs to manage and coordinate care in a manner consistent with Devoted's values of treating every member like family. Medication plays a crucial role in helping people manage chronic conditions, but in many cases effective medication management is limited by barriers to access, uncoordinated prescribing, or a failure to use the most effective evidence-based medication therapies. For example, despite a mountain of evidence, less than 1 in 10 people with congestive heart failure (CHF) are prescribed evidence-based medications proven to save lives and prevent hospitalizations.
Devoted's clinical pharmacy team aims to support improved therapeutics for Devoted members. It supports this work in three ways:
- Conducting medication reviews (via direct member engagement and pharmacist consult support for other clinicians)
- Virtual pharmacist-led or clinician-led care via telemedicine to close therapy gaps
- Coordinating care for changes in therapy by counseling members and engaging prescribers and pharmacies
This work is done in service of a number of goals. The clinical pharmacy team drives Devoted's quality and Medicare Stars performance across multiple programs such as:
- SUPD (Statin Use in Persons with Diabetes)
- SPC (Statin Use in Persons with Heart Disease)
- MTM (Medication Therapy Management)
- TCC (Total Cost of Care)
- Polypharmacy
During appointments with our pharmacists and clinicians, medication opportunities are identified.That's where Pharmacy Care Coordinators (PCCs) step in. PCCs ensure recommendations reach the outside provider, follow up with pharmacies on claims issues, pick-up confirmations, or discontinuations, and keep the member informed of any changes along the way.
As a PCC, you will partner with our Clinical Pharmacy and Operations teams on outreach, coordination, and follow-up to provide best in class care to our members, as part of a mission-driven, team-oriented, joyful culture.
Responsibilities and Impact will include:
- Supports clinical outreach campaigns by making outbound calls and utilizing portal chat to members to schedule appointments with the pharmacist team to address clinical gaps or medication concerns
- Supports cost of care efforts by reaching out to providers and members to offer lower cost alternative medication options
- Manages an assigned caseload which may require provider, pharmacy, and/or member follow-up to address medication recommendations, concerns, or questions
- Independently oversees assigned cases and outreaches with timely follow-up, clear next steps, and thorough documentation
- Helps members navigate their Devoted Health benefits
- Participates in sprints, pilots, and special projects as they launch
- Collaborates with the broader pharmacy team to support shared goals and coverage needs
- Assists with training and shadowing when new team members join the team
- Participates in team and pharmacy department activities and responsibilities
Required skills and experience:
- At least 3 years of work experience in health care or retail pharmacy
- Active National PTCB Certification
- Comfortable making a high volume of outbound calls to members, providers, and pharmacies
- Comfortable managing daily responsibilities and tasks independently
- Ability to shift between tasks, projects, and priorities frequently

- Comfortable learning new software and adapting as tools and workflows evolve
- Adaptable and flexible, with a willingness to take on new or shifting tasks positively
- A true team player who brings new ideas to the table, works through solutions with others, and lifts up their teammates
- Comfortable speaking with physicians, pharmacies, and patients, with a genuine passion for our members
- Thrives in a fast-paced and dynamic environment
- A transparent communicator about your work, what's going well, and what's not, with a thoughtful eye for improving processes
Salary range: $23.00 - $30.00 hourly

The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. Once the interview process begins, your talent partner will provide additional information on the compensation for the role, along with additional information on our total rewards package. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
