职责
关键成果 有效高效的远程康复患者护理管理：使用ATI的虚拟远程康复平台，按照物理治疗师制定的计划，对来自不同地理区域的患者进行各阶段的物理治疗；定期进行客观再评估，跟踪目标达成进度，或根据患者的康复轨迹与监督物理治疗师调整目标；实施符合ATI最佳实践指南和循证护理的护理计划，以实现可预测的结果；协助监督物理治疗师有效管理其患者群，涵盖每位患者护理计划的各个方面；按时完成治疗医生、康复护士或保险公司、病例管理员和医生转介要求的日常记录/笔记和其他表格；提供有价值的患者教育以促进康复；并定期更新家庭锻炼计划，达到或超过生产率期望；实施有效的基于团队的护理模式以优化结果：与监督物理治疗师和其他支持人员有效合作；在监督物理治疗师的指导下发展技能，为每位患者提供无缝体验；是虚拟治疗护理团队的重要成员。

Overview
The Telerehab Physical Therapist Assistant is responsible delivering effective treatment of patients, based on established outcomes-centered plans, and partnering with the team’s supervising Telerehab Physical Therapist(s) to ensure timely, progressive and high valued care for those we serve leveraging ATI’s industry-leading telerehab platform.
The successful Telerehab Physical Therapist Assistant is a critical member of the Telerehab Care Team. Collaboration and encompassing a “team-player” mindset will facilitate alignment to ATIs broader mission as we are all “One ATI.”

This is a remote position for U.S. based employees. Candidates who reside in the western United States with availability to work U.S. Pacific time zone hours are highly preferred.
Candidates must be Licensed Physical Therapist Assistant in home-state, and eligible for licensure across multiple states.
Responsibilities
Key Outcomes Effective and efficient care management of telerehab patients: Treats patients using ATI’s virtual telerehab platform throughout all phases of physical therapy recovery, with patients across varied geographic locations, as established by a licensed Physical Therapist Implements regular objective reassessment points to track progress towards goal achievement and/or works with the supervising Physical Therapist to adjust goals based on patients’ recovery trajectory Implements care plans aligned to ATI Best Practice guidelines and evidence-informed care, to achieve predictable outcomes Supports the supervising Physical Therapist in managing his/her Caseload effectively, across the spectrum of each patient’s plan of care Completes daily documentation/notes and other required forms as requested by treating physicians, rehab nurses or insurance companies, case managers and physician referrals in a timely manner Provides valuable patient education to empower recovery; and regularly updates home exercises to Meets or exceeds productivity expectations Implement effective team-based care model to optimize outcomes: Partners effectively with the supervising Physical Therapist and other support staff Develops skills with mentorship from the supervising Physical Therapist to deliver a seamless experience for each patient Is a valuable member of the virtual treatment care team, building strong morale and teamwork Communicates the roles of the clinical care team to each patient
Qualifications
- Associates Degree from an accredited Physical Therapy Assistant program.
- Licensed Physical Therapist Assistant in home-state, and eligible for licensure across multiple states.
- 3-5 years of experience as a licensed Physical Therapist Assistant (PTA).
- Must be skilled with technology and able to provide effective verbal, visual and written feedback using a virtual platform.
- Ability to multitask across multiple cases and episodes of care across various geographies.
- Work with integrity, managing care responsibility and in compliance with state practice acts and payer requirements.
