职位描述概要

Job Description Summary
The Palliative Care Medical Coder focuses on converting palliative and hospice clinical documentation into standardized billing codes so Stillwater Hospice can be paid correctly and compliant with all payers and regulations.Essential Job Functions/Responsibilities
- Review palliative/hospice visit notes, consults, telehealth, advance care planning encounters, and procedures, then assign accurate ICD-10-CM diagnosis codes and CPT/HCPCS procedure/E&M codes.
- Strong understanding of medical terminology, anatomy, physiology, and palliative care documentation standards.
- Ensure codes support medical necessity, hospice/palliative eligibility, and align with CMS, MAC, and commercial payer rules, including modifiers and place-of-service requirements.
- Query clinicians when documentation is incomplete or unclear and provide feedback to improve documentation quality.
- Work coding work queues in the EHR/billing system, resolve edits/denials, and support audits and revenue-cycle reporting.
- Maintain HIPAA compliance and keep up with coding guideline changes specific to hospice, palliative consults, and advance care planning.
- Work independently with set productivity/quality targets.
- Use our EHR, billing software, and secure remote-access tools reliably.

- Bachelor's degree or Associates degree in Health Information Management or related field preferred.
- CPC (AAPC), CCA or CCS-P (AHIMA) certification plus direct hospice/palliative experience.
- Experience with Medicare hospice and palliative billing rules and denials.
- Excellent attention to detail and analytical abilities.
- Proficiency with EMR and coding software systems.
- Clear communication and collaboration skills.
