Position Purpose: Supervises community and resource coordination activities to ensure members are connected to local resources and are being supported throughout care/service. Supervises day-to-day activities of community and resource coordination team members and issues related to members.
Key Details: Must be authorized to work in the U.S. without the need for employment-based visa sponsorship now or in the future. Sponsorship and future sponsorship are not available for this opportunity, including employment-based visa types H-1B, L-1, O-1, H-1B1, F-1, J-1, OPT, or CPT.
***This candidate must reside in the state of Texas.***
- Monitors and reviews all community and resource coordination activities including member outreach and ensuring members are connected to local resources throughout care/service
- Assigns community and resource coordination tasks to the team based on state and contractual requirements and member needs
- Works with community and resource coordination and care management team to ensure effective, appropriate care of the member and addressing member needs
- Ensures compliance with state and regulatory guidelines and standards
- Educates and provides guidance and/or resources for community and resource coordination team to address member requests/needs
- Evaluates community and resource coordination team’s performance and provides feedback regarding performance, goals, and career milestones
- Provides coaching and guidance to community and resource coordination team to improve community resources, outreach services and connecting members to community resources to provide necessary care in a cost-effective manner
- Assists with onboarding, hiring, and training community and resource coordination team members
- Performs other duties as assigned
- Complies with all policies and standards
Education/Experience: Requires a Bachelor's degree and 4+ years of related experience.
Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.Pay Range: $63,600.00 - $114,600.00 per year
