Make a meaningful difference for mothers, babies, and families across Illinois. As a Care Coordinator Doula, you'll help support Humana's IL Medicaid members throughout their pregnancy, childbirth, and postpartum journey. Working with a multidisciplinary team of nurses, social workers, behavioral health professionals, and care coordinators, you'll help ensure members receive compassionate, personalized care that improves health outcomes and addresses barriers to care. This is an opportunity to combine your doula expertise with care coordination, advocacy, and community engagement to positively affect the lives of families during one of their most important life experiences.The Care Coordinator Doula's responsibilities include:

  • Ensure pregnant and postpartum Medicaid members are receiving appropriate services in the least restrictive setting to achieve and/or maintain optimal well-being by assessing their care needs.
  • Identify and resolve barriers that hinder effective perinatal and postpartum care.
  • Plan and implement interventions to meet care needs.
  • Coordinate services, monitor, and evaluate the case management plan against the member's personal goals.
  • Ensure the member is progressing towards desired outcomes by continuously monitoring through use of assessment, data, conversations with members, and active care planning.
  • Assess and evaluate members' needs via telephonic and face-to-face assessments and evaluations, to coordinate services and address enrollee needs.
  • Assist members to remediate immediate and acute gaps in care and access.
  • Provide information to enrollee related to MCO requirements, services, and benefits.
  • Provide enrollee with information and/or referrals to community resources.
  • Guide members/families towards resources appropriate for their care. Services are driven by facilitating interactions with other payer sources, providers, interdisciplinary teams, and others involved in the member's care as appropriate and required by our comprehensive contract.
  • Visit Medicaid members in their residences – up to 25% local travel

Required Qualifications

  • Illinois Medicaid-Certified Doula
  • Must reside in Illinois.
  • 1+ years of experience in health care.
  • 1+ years of case management/care coordination experience.
  • Intermediate to advanced computer skills and experience with Microsoft Word, Excel, and Outlook.
  • Experience using a variety of electronic information applications/software programs including electronic medical records.

Preferred Qualifications

  • Experience working with Medicaid recipients.
  • Experience providing doula services to individuals and families during pregnancy, childbirth, and the postpartum period.
  • Knowledge of community health and social service agencies and additional community resources.
  • Previous managed care experience.
  • Bilingual.

Additional Information

  • Workstyle: This is a remote position that will require you to travel.
  • Travel: Up to 25% local travel for collaboration and face-to-face meetings as well as field interactions with staff, providers, members, and their families.
  • Typical work days/hours: Operating hours areMonday - Friday 8:00 AM - 7:00 PM CST but core hours for this position are primarily Monday – Friday, 8:00 AM – 5:00 PM CST. May need to be flexible with work schedule based on business needs.

Driving
This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
TB
This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. 40Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
