居家护理协调员

Objective To coordinate a seamless, safe and efficient home care service that communicates a legacy of care and offers impeccable patient experience.
Duties & Responsibilities: Note that this is a guideline, not a comprehensive list of duties.

• Provide information regarding medical care, follow up and interventions.

• Provide support to medical staff for seamless home-care delivery.

• Perform administrative and official duties.

• Coordinate safe, efficient and timely delivery of home-based medical care and hospital consults where relevant.

• Ensure compliance with hospital and legal requirements

• Verify insurance information of patients and confirm payments are done.

• Maintain a healthy and cooperative environment with both patients, staff and relatives.

• Make reports and database of patients, inventory and other relevant information

Qualifications & Requirements

• KRCHN

• BScN added advantage

• Held leadership roles for at least 3 years

• Work experience of 7 years in an acute care hospital

Essential Skills and Proficiencies

• Knowledge of various clinical techniques, diseases, medical terminologies and other medical functions

• Experience in handling medical and administrative duties with minimal supervision.

• Ability to understand and respond to urgent and emergent issues.

• Ability to perform duties with optimum utilization of resources.

• Excellent verbal and written skills

• Excellent listening skills

• Strong organizational and multi-tasking skills

• Ability to counsel patients and relatives of all ages

• Knowledge of latest trends in healthcare

• Well organized, detail-oriented and confident personality

• Strong Interpersonal and time management skills

• Competency in MS Office

• They should be a person with very high standard of integrity.
